NCM utilizes a third party
administer to contract with Independent Contractors for
their services to accommodate the transportation service
needs of our clients. We are a contracting company with the
Subcontracting Concepts Inc, SCI.
SCI ensures that all Independent
Contractors affiliated with them obtain licenses, permits,
and registrations required by state law to be considered a
legitimate business. In addition, they provide affiliated
Independent Contractors with On-the Job Accident Policy
coverage, Off-the-Job Accident Policy coverage, Disability
Benefits, Accidental Death and Dismemberment Policy
coverage, tax services, referral services, verification of
information services, and other beneficial services. For

At NCM, safety is our primary goal.
Independent Contractors who provide courier services must
carry primary insurance with a minimum of 100/300 liability
coverage and meet the following Three-Year Motor Vehicle
Record requirements in order to qualify for affiliation with
NCM:
- a maximum of two moving violations , or a maximum of
one moving violation and one accident
- no record of an at-fault accident resulting in a
fatality or serious injury
- no violations for driving under the influence of
alcohol and/or drugs
- no refusal to take a test for being under the
influence of alcohol and/or drugs
- no violation of reckless driving or careless
endangerment
- no violation of reckless homicide, involuntary
manslaughter
- no violations of operating a vehicle while under
suspension or revocation
- no violations of use of a motor vehicle in the
commission of a felony
- no violations for speed exhibition, contest or drag
racing
In addition to a clean
driving record, please detail your operation’s hours of
availability and other information that is pertinent.
- DENTAL BENEFIT
PLAN – HUMANA
Employees participating in
our HumanaDental plan may access their account via
HumanaDental’s website. Participants may only view their own
information or information for their dependents that are
under the age of 18. Spouses can’t view each other’s
information or information pertaining to dependents 18 years
of age or older. Once registered online, active participants
in our plan may check the status of a claim, request an ID
card, check benefit eligibility, locate participating
dentist, and much more.
To register, members will
need the following information to set up their personal
account online:
- First name, last name
including suffix if applicable
- zip code
- date of birth
- valid email address
- HumanaDental Member ID number
located on your ID card
One you have your information
ready, go to
www.humanadental.com and click on the “member” tab to
register. Click on the “First time users: How to
register” and the website will guide you through
the registration process. Once registered, you would login
under the member ID and password you established under the
registration process so remember to write this information
down when you register.
As a registered HumanaDental member
your online access provides you with all of the following:
- Provider Tools – listings of
dentists and EyeMed vision providers who are
participants in HumanaDental’s network. Please confirm
their membership is current prior to receiving any
services.
- Access general information
about your dental plan.
- View summary of benefits
including co-pays and deductibles.
- View coverage details
documents which provide comprehensive benefit
information and any required disclosures accompanying
the documents.
- View your year-to-date
deductibles and maximums, the dollars applied to your
out of pocket expense as well as any waiting periods
that may be applicable.
- Request to receive a
replacement ID card by mail.
- View your claim history
including date of service, status of claim, total
charged, amount Humana paid, your estimated out-of
–pocket charge and other details of each individual
claim.
You will also receive informational
emails from HumanDental via the email address you provided
when you registered. You may also send and receive messages
via your personalized page on
HumanaDental’s website.
